Submit this form with your information to request opt-out and deletion from MyLife.com. Requests are processed within 7 business days. Any Reputation Profile published on our website that matches your information, will be permanently removed.

The information you provide is to make sure MyLife removes the correct record(s), particularly if there are multiple profiles or records under the same name or a similar name.

If you include an email address on the form, we will send you an emailed confirmation when the deletion is complete. We will also 'unsubscribe' the email and delete any accounts you may have registered using that email.

To request a copy of your information, contact MyLife Customer Care by email. Requires 30 days processing time and an email address for delivery.
